Our receptionist will be pleased to connect you to the appropriate program or schedule an appointment for you. What to Bring Please bring the following to your first appointment at Lake Shore Behavioral Health: Photo ID Insurance Card Proof of Address (i.e. utility bill etc.) Cash, check or credit card (to pay insurance co-pay or cost of first appointment) If you do not have health insurance, you should also bring the following: Birth Certificate Social Security Card Recent pay stubs, if employed Please note that Lake Shore Behavioral Health accepts most health insurances and a sliding fee scale is available. No one is turned away due to inability to pay. Emergency Contact Information In case of emergency, please contact Crisis Services of Erie County at 716-834-3131. Crisis Services offers a 24-hour hotline specializing in providing emergency services, including suicide prevention and emergency outreach. If the risk of harm is immediate, please call 911 or your local police for immediate assistance.
