  Intensive Mental Health Services

Lake Shore Behavioral Health's Intensive Services programs are comprised of the Homeless Services Program, Assertive Community Treatment Programs (ACT) and Case Management Program.

Homeless Services
The Homeless Services Program provides a wide range of support and treatment services to homeless persons with mental health problems. Services include outreach, crisis intervention, mental health and substance abuse counseling, housing assistance and linkage to other needed services. Through the use of a mobile mental health outreach team, the agency is able to offer services on-site at homeless shelters as well as to individuals on the streets. The program operates 132 supported housing units, placing individuals in scattered site apartments and providing ongoing support to residents.

Assertive Community Treatment (ACT) Programs
Lake Shore Behavioral Health operates two ACT teams - Aspire and Impact. Each team can serve 68 individuals. The ACT teams provide services for individuals with severe and persistent mental illnesses who have not been able to be served through traditional outpatient mental health programming, and have a significant history of hospitalizations for mental health reasons. ACT is a comprehensive, team-based approach to treatment, with a strong emphasis on recovery principles. Services include case management, medication management, vocational counseling and placement, family counseling and psycho-education, mental health and substance abuse counseling and wellness management education. Services are provided in the community, in the consumers' natural settings.

Blended Case Management
For individuals who require a more intensive level of case management than that offered through traditional outpatient programs, the Blended Case Management program is available. The program accepts both hospital discharge referrals as well as outpatient clinic referrals and operates within the Erie County Single Point of Entry.
